Challenge
IDPH needed a means to collect valuable salary and benefit information of employees from ninety-four local health departments located throughout the State of Illinois.
Role
As the lead designer, responsible for:
- Interface Design
- Site Architecture
- HTML Coding
Results
This clean looking interactive survey allowed IDPH to gather data from a database and use that data to create a comprehensive report detailing the compiled information that will be distributed to each of the local health departments.
